I have a 2013 toyota minivan that is most likely totaled due to cosmetic damage and mileage. Thinking about buying it back and driving it as it is without fixing the damage. I know it would need a salvage title how does that work in NC would it need a special inspection to put it back on the road

Has NC gone to emissions testing only? If not, figure out it’ll pass a full inspection before you buy it.

are there special inspectors/inspection stations to check salvage vehicles? Also my area does not have emission testing so would I need it?

IDK, but also make sure you can get insurance on it without performing the repair.

I really have no idea how to check these things, my expertise is in seeing how to get screwed and not in how to avoid the screwing.

I did this with my wife’s 2005 Honda minivan, and also with my 2012 Subaru Forrester. Brought back both of them. No special inspection required. And no issues getting them inspected each year.
Check with your insurance company. My insurance company did not care when I asked him about it, which I was surprised to hear. But I think the only potential issue is they may not offer you comprehensive coverage (coverage for your own vehicle if you do something that is your fault, or if the other driver is not insured) anymore.
On both my vehicles it was a seamless process. I did not have to go to the DMV or switch plates or anything. The insurance company may have sent me some papers to sign, and then they took care of everything. They may have sent me a new title in the mail. I can’t remember.

All counties have safety inspections until the vehicle is 30 years old.

19 counties also have emissions inspections until the vehicle is 20 years old. Here’s the list:

Some other exemptions in addition to being 20+ years old:
