Problem with a friend's AR. Possibly related to the safety.

Took a friend to the range to shoot his new AR for the first time (it was built at a LGS). Every time it fired, it would eject the round, chamber a new one, then not fire. After much experimenting, I discovered that if he toggled the safety between each round, it would work as expected. Maybe the bolt was activating the safety when it hit the buffer tube?

After putting a couple hundred rounds through it this way, it seemed to mostly work okay. Anybody have any ideas?
 
would it click when you pulled the trigger the second time?

Pull the upper off and take a picture of the inside of the lower.
 
Pulling the grip to check for safety spring/detent, and removing/reinstalling the FCG to make sure everything is correct, are both two fairly simple steps to take.
 
Disconnector spring missing or upside down?
 
He took it to an indoor range and was still having the problem. Their gunsmith was going to take a look at it. It's his first gun so he feels better having someone else look at it for him.
 
Buddy had the same problem with an Anderson LPK. Had to manually reset the trigger by pushing it forward. Tiny, tiny burr on the selector that was remedied with a rattail in 8 seconds.
